I have a Ford factory puck system in my truck with the B&G Companion hitch. Had similar issues a year ago and scared the bejeesus out of me at the time. Finally figured out that the connection point between the base and truck was not tight enough and the rust inside the tubes where the hitch slides in was allowing slippage. I ended up tightening each bolt slightly to get the cotter pin in the crown bolt to the next hole and the problem went away.
It makes it more difficult to remove the hitch, as now I need a wrench to loosen the bolts and take the base out, but I feel better knowing its tight enough to not slip.
