I think it's a matter of how hard they want to work and if they happen to have the right parts in stock. I imagine there might be different size nuts for different wheels and since these might require more work, then a lazy tech might be able to talk you out of them.

When I got my new tires for the truck, I specifically asked for metal stems after giving them the year/model truck I had and the size of rim and five tires worth of business (America's Tire this time). Since they provided my new spare they have the parts for that too. I had no issues.

You'll need to get to a more capable tire store to get these installed and they will probably charge for labor if you aren't getting any other business from them. I think it's a good idea to consider putting a monitor on the spare, too, hence it will need a metal stem, too.
